Peabo Quarry explosion caused by human error - Captain Smart

The explosion of the Peabo Quarry in Nsawam-Adoagyiri in the Eastern Region, with catastrophic consequences in December, 2015, on Friday was caused by human error.

This is according to report by the Ministerial Committee set up to investigate the incident.

Captain Smart host of Adom FM’s morning show Dwaso Nsem revealed this on the Fabewoso segment on the show.

The blast from a quarry site belonging to A.K.Y Mining Services Limited along the Adeiso road occurred as a result of a naked fire getting into contact with ammonium dynamite.

The devastation caused many injuries while substantial properties were lost through the accident.

Captain Smart said even though the Ministerial Committee set up to investigate the explosion recommended a ten-year ban on the operations of A.K.Y Mining Services Limited, it is still operating.
